I offer light case management for those who need additional support around disability benefits, accessing services, or school advocacy/support. I also offer care co-coordination for existing clients for complex cases. This is based on a flat hourly fee that includes consult and needs assessment in first appointment, followed by any research or documentation for case management. For care co-coordination this consists of attending meetings outside of therapeutic hours, creation of tools and documentation for schools/home/work use, and any psychoeducation that may be provided.
